HEALTH SERVICES

Health Information

The Mount Baker School District employs a certificated nurse who rotates between district buildings.  In the event you need to speak directly with the nurse, contact the school where your child attends, the secretary will assist you.

Health Screenings

We provide state mandated annual vision and hearing screenings and referrals for students in the following grades: Kindergarten, 1st, 2nd, 3rd, 5th, and 7th.  We also provide screenings as needed for any child showing symptoms of possible loss in auditory or visual acuity and for special programs health assessments.

Immunization

Immediately upon registration and when the state immunization requirements indicate, each student’s parent/guardian must provide a signed certificate of immunization form documenting full immunization, proof that a schedule of immunization has begun or a signed certificate of exemption.  Students may be exempted for medical, philosophical or religious reasons. Contact District Nurse, for more information and/or form @ 383-2012.

Medications

If a child requires oral medication at school, either over-the-counter or prescription medication, a medication administration form must be filled out and signed by both parent/guardian and health care provider/dentist.  For the safety of everyone, all medication must be delivered to school by an adult and needs to be in its original container.  This is especially important information to remember if your child should go on an overnight field trip or for other school activities.

Whatcom Alliance for Healthcare Access—WAHA

The Whatcom Alliance for Healthcare Access (WAHA) is a nonprofit community service that partners with our school district to offer families free, impartial information on state and commercial health insurance programs.  WAHA’s insurance counselors are available to help families look at their options and complete state application forms.  Call 788-6594  or email them at waha@hinet.org

Participation in Athletics

All students participating in athletics are required to have a physical examination and health insurance before participating in Washington Interscholastic Activities Association (WIAA) programs.

Purchased Student Insurance (call Athletics office @ 383-2022  for more information)

Enrollment information is sent home with each student in the fall.  Contact the District Athletic Office at 383-2022.
