In order to be eligible to participate on the first day of practice, you must complete the necessary paperwork. If you plan on playing sports in the fall, make sure you complete and turn into the junior high office the following forms:

Mount Baker School District Athletic Code: This form outlines the rules and expectation for student athletes in the Mount Baker School District. |

Eligibility Packet - Print, read, sign and turn in to the MB athletics office. |
|
MB Physical Form - Take this to your physician. |
In addition, every athlete must purchase an ASB Card in order to participate in athletics. ASB cards can be purchased in the high school or junior high office.
