Incoming Sixth Grade Students


Washington State Law WAC 248-100-166 requires that all children attending public school have specific required immunizations.

The Tdap (Tetanus, Diptheria, acellular pertussis) is a new vaccine which the State is REQUIRING schools to have in their records.  One does is required IF student is in grade 6 AND if student is greater than or equal to 11 yeards old.  (if the student is not 11 years or older, then when they do turn 11 and receive the Tdap.  The 6th grade students also need two doses of varicella (chickenpox) vaccination or history of disease verified by health care provider.  It is the parent/guardian’s responsibility to provide this information to the school secretary). 

Therefore, BEFORE THE FIRST DAY OF SCHOOL, it is the parent/guardian’s responsibility to contact the school secretary with the name of the students, the name of the vaccines and the date the vaccines were received.  Parent/guardian may also request the Dr./health provider fax immunization information to the specified school.
