Sports Physical Clinics
at PeaceHealth Medical Group Orthopedics & Sports Medicine
June 28th, July 27th, & August 16th
Visit will consist of complete set of vital signs: age, height, weight, pulse, BP, visual acuity, screening exam per form.
Parameters:
- Sports Physical Clinics are for students entering grades 6 and above.
- Patients to arrive with their portion of Sports Physical Exam form completed, and a list of medications and allergies.
- Understanding that this is a screening exam for participation only; no specific medical concerns will be addressed or treated.
- Patients with chronic conditions will require clearance from another medical professional.
The cost of this visit is $75 and will be billed to patient after the visit. This will not be submitted by our office to patients’ insurance for reimbursement. Please reach out if you have any questions regarding payment.
